A worker is sweeping the floor behind a forklift. The driver of the forklift starts moving backward and runs him over.

There are issues with the forklift design. Because of a special attachment in the front, extra weight had to be added to the rear. Weights were stacked on the back of the machine which raised the truck's propane tank to a point where it interfered with the driver's vision. Simulations showed that if the worker sweeping the floor was 4 feet to the rear of the machine as he said he was, the driver could have missed seeing him.

However, a supervisor over 200 feet behind the forklift, said he saw the machine move 20 feet before it struck the sweeper. We animated the situation to show that the supervisor could not accurately determine the distance the forklift traveled.
